Privacy Policy Details

We collect personal information necessary to provide dental care, including: name, contact information, date of birth, health card number, dental and medical history, diagnostic images and records, insurance information, and treatment records. This information is collected directly from you (or your legal guardian) during consultations, appointments, and through patient intake forms.

Your personal health information is used to: provide and coordinate your dental care, communicate with you about appointments and treatment, process insurance claims on your behalf, comply with legal and regulatory requirements, and improve the quality and safety of care provided at our practice.

We maintain physical, administrative, and technical safeguards to protect your personal health information from unauthorised access, disclosure, or misuse. Electronic records are stored on secure, encrypted systems. Physical records are stored in locked, access-controlled areas. Staff receive training on privacy obligations and information security practices.

We do not sell, rent, or share your personal health information with third parties for marketing purposes. Your information may be shared with other healthcare providers involved in your care (with your consent), insurance companies for the purpose of processing claims, and regulatory bodies as required by law.

You have the right to: access your personal health information and request corrections, withdraw consent for certain uses or disclosures (subject to legal limitations), file a complaint with the Information and Privacy Commissioner of Ontario if you believe your privacy rights have been violated, and be informed of any breaches involving your personal health information.

We retain your personal health information for the period required by law and by the standards of the Royal College of Dental Surgeons of Ontario. After the retention period, records are destroyed securely using document destruction services.

To request access to your personal health information or to make inquiries about our privacy practices, contact our Privacy Officer at the practice. We will respond to your request within 30 days, in accordance with PHIPA requirements.

If your concern is not resolved, you may file a complaint with the Information and Privacy Commissioner of Ontario (IPCO) at 1-800-387-0073.
